What Is a Prescription Prepayment Certificate (PPC)?

A PPC lets you pay £114.50/year for unlimited NHS prescriptions. It's great if you take 12+ items per year. But for cheap generics, a private prescription might be cheaper.

A Prescription Prepayment Certificate (PPC) is an NHS scheme that lets you pay a fixed annual or quarterly fee to cover all your prescription charges — no matter how many items you collect. The annual PPC costs £114.50, which works out as cheaper than paying the standard £9.90 charge if you collect 12 or more prescription items per year, or roughly one per month.

How Much Does a PPC Cost?

The Prescription Prepayment Certificate costs £114.50 per year or £32.05 per quarter in 2024/25. You can pay the annual fee upfront or spread it over ten monthly instalments. The quarterly option costs more per year — £128.20 if renewed every three months — but suits patients who want to trial the scheme before committing. Both options are available directly from the NHS Business Services Authority (NHSBSA) online or by phone.

When Does a PPC Save You Money?

A PPC saves money when you pay for more than 12 prescription items per year. Each NHS prescription item costs £9.90; 12 items would cost £118.80 — already more than the £114.50 annual PPC. In other words, if you collect one or more items per month on average and pay the prescription charge, a PPC is almost always worth buying. NHSBSA data shows 1.26 billion items were dispensed in England in 2024/25, so multi-item patients are extremely common.

Who Qualifies for Free NHS Prescriptions Instead?

Before buying a PPC, it is worth checking whether you already qualify for free prescriptions. Around 89% of all items in England are dispensed without charge, under exemptions covering: patients aged 60 or over, those under 16 (or under 19 in full-time education), pregnant women and those who have given birth in the last 12 months, people with certain medical conditions including diabetes, epilepsy, and hypothyroidism, and people receiving certain benefits. If you qualify, there is no need for a PPC.

When Is a Private Prescription Better Than a PPC?

A PPC covers unlimited NHS items, but it costs £114.50 upfront per year. For a patient on only one or two cheap generic medicines, a private prescription at £5.50 per item may be cheaper. One monthly private prescription costs £66 per year — £48.50 less than a PPC. Two items monthly costs £132 per year — still £17.50 less than the annual PPC. Only once you collect three or more items per month does a PPC clearly beat private prescriptions, at which point the NHS route becomes the more economical choice.

How Do You Buy a PPC?

You can apply for a PPC online at the NHSBSA website, by phone on 0300 330 1341, or by completing a FP95 form at your GP surgery or pharmacy. The certificate is typically issued as a card or can be shown digitally. Once active, you present it at the pharmacy alongside your prescription and pay nothing for each item collected. The certificate covers you for the full term, and there is no limit on the number of items or the cost of the medicines.
